go to producent site http://www.leadtek.com/eng/support/download.aspx and 3d graphics -> agp cards -> find your model ;-] probably some from gf4 series. i cannot find that model what you wrote here ;]

You probably need to switch the refresh rate of the monitor from monitor settings or VGA settings to the highest possible..
CRT you have??
It could go up to 120 or 240hz in some cases,

"ghosting" for me is related only to refresh rate. try lower the resolution, and check with what refresh rate works monitor. also check that old/newer graphic card
maybe rly its only overheated.

as the matter of fact, i have tried everything, but couldnt solve the problem , so my conclusion is that this graphic card is dying/or not suitable for this pc config.
So now, i replaced it with this PC's native one , its WinFast A80B T 64Mb, and its working but i can't find the proper driver for this one on net...
