When did this game get severely broken?

Played a FW last night and during the game there was many instances of players just appearing in front of me from BFG jumps or from around corners. Then today playing some 1v1's and the sounds are all f**ked. I could hear gaunt sounds being left where the player once was, and the ambient gun sounds seem different, less obvious.

When did this game become so broken? Who's responsible?

Why would easy release something that's totally incomplete?

I don't mind advances in anti cheat software but when it's not even finished and ruins the game why release it?

This "antiwallhack" is ruining the game on a competitive level and seriously needs left out of the mod till its fully working right, without spoiling the game.

I've had a break from the game since 2.3, I think I've only played a handful of games on that release till now.

What I don't understand, why is everyone fine with this broken game now? Shouldn't something have been done ages ago if it was always like this?
