But to save me trolling through who knows how many messages on the forum, can someone please explain how I use the grapple please and how I set it up. Bind it to a key or mouse key please.

either type /bind key +button5 in the console or edit/add the command to your config file (q3config.cfg) located in your quake 3\excessiveplus directory.
