perhaps your connection cant handle the rate and max_packets
try max_packets 40 and rate 15000. then work your way up.

mhh u play on dynamiclight instead of normally vertexed light right ? ... when i do so it always feels a bit strange even with a new 512mb gc ... just for a try ... seta r_vertexLight "0" -> "1"
.. but u got "how i see" seta r_dynamiclight on 0 so what does ur Quake take ? dynamic or vertex ? 
... just for testing leave it at 0 and vertex on 1
seta snaps "100" ? ... set it to arround 40 how said 
Maybe there are some of the r_ext settings which makes trouble in combo with your gc ... just watch out for the guide or manufacturer's Website to see which extensions or card got and which not ... but your card should be able to get all of them ^^
try a bit arround there and come back with some good feedback
