Yo, dunno what Alt+Enter suppose to do, I have never used it, but you need also r_customaspect 1.

Alt+enter is working as a substitution for minimization the quake window when alt+tab doesn't work; You resize fullscreen to windowed mode so you can check other stuffs not related to quake 3.
PS. Yes, I know "q3minimizer" but it sucks monkey balls, never worked well for me.
