you specify it when creating a server, like this:
quake3.exe +set fs_game excessiveplus +set vm_game 0 +dedicated 2 +exec server.cfg

edit:
also, make sure you have specified desired master server in your server.cfg, like this:

seta sv_master1 "master3.idsoftware.com"
seta sv_master2 "master.quake3arena.com"
...
seta sv_master4 "..."

as for sv_master5 - the mod forces it to point to e+ master server master0.excessiveplus.net
