Does it 'lag' when you fire grenade or when grenade explodes?

also you say when sp is ok.. you mean singleplayer with e+ loaded or modless? and when you connect mp is it e+ you connect to or have you tried an unmodded server? thought behind this is eliminating any e+ cvars from your problem.

my guess is you have new set of drivers for your gpu? one of the r_cvars is causing conflict. if all else fails rollback drivers or at least check settings in your card for running under q3a.

while grenade is in the air, its laging. But same on OSP and e+ mod.

I believe that God left certain drugs growing naturally upon our planet to

Try disabling threaded optimization for quake 3 in your gfx drivers. You say it doesn't happen in other mods (and cfgs?) but might be worth a try anyway.
